Registered Dietitian or Certified Nutritionist PACE Program Specialist

HEOPS is an integrated care company and PACE Program (Program of All Inclusive Care for the Elderly) dedicated to providing comprehensive care to our members. We are seeking a Registered Dietitian or Certified Nutritionist to join our team as a key member of our interdisciplinary care team (IDT).
The successful candidate will have a strong background in nutrition and experience working with elderly populations. They will be responsible for evaluating participants' nutritional status, assessing their food preferences and cultural factors, and developing personalized nutrition plans. The ideal candidate will be able to work effectively within our IDT and communicate with participants, their families, and other healthcare professionals.
Key Responsibilities:
Evaluate participants' nutritional status and develop personalized nutrition plans
Assess participants' food preferences, cultural factors, and environmental conditions
Participate in the development and revision of participants' care plans as a member of the IDT
Educate participants' families or caregivers about therapeutic diets and nutrition plans
Re-evaluate and revise nutrition plans as necessary to ensure continued adequacy and appropriateness
Requirements:
Bachelor's degree in nutrition or a closely related field
Licensure or certification for dietitians or nutritionists
At least 2 years' experience in nutritional counseling and education
At least 1 year's experience working with elderly populations
Ability to work effectively within an IDT and communicate with participants, their families, and other healthcare professionals
