Key Responsibilities:

  • Assess and develop discharge plans for adult and geriatric populations, ensuring a smooth transition from hospital to home or other care settings.
  • Collaborate with multidisciplinary teams, including physicians, nursing staff, and other healthcare professionals, to promote care coordination and ensure comprehensive patient care.
  • Provide emotional, social, and financial support to patients and families, addressing complex family dynamics and psychosocial issues.
  • Mobilize community resources to meet identified patient needs, ensuring timely and effective support.
  • Document all interventions and services provided, maintaining accurate and up-to-date records.
  • Prepare care plans for forensic patient court hearings, as required.

Requirements:

  • Master's Degree in Social Work/Counseling from an accredited institution.
  • Current licensure in the State of Maryland – LMSW/LGPC required; LCSW-C/LCPC preferred.
  • Minimum of 1 year of experience in a healthcare setting, with hospital experience preferred.
