Key Responsibilities:

As the Recreation Program Facilitator, you will design, organize, and implement skill-building, life enrichment, and recreational activities for adults with disabilities within community settings. You will also provide transportation for participants using Rise vehicles. Your role will involve working across both the Fridley and St. Louis Park programs, with an equal distribution of time between the two.

  • Develop and curate resources and curriculum for day services, contributing to a shared library for all Rise programs.
  • Create and manage monthly and weekly activity calendars.
  • Oversee the inventory of materials required for activities.
  • Collaborate closely with the on-site Rise Supervisor and Case Managers to ensure activities are tailored to individual needs and align with program objectives.
  • Establish and nurture effective partnerships with community organizations to create volunteer opportunities.
  • Provide individualized support using a person-centered approach, exemplifying best practices.
  • Lead by example in delivering exceptional service to individuals, their families, and all stakeholders involved.
  • Assist individuals in transferring and repositioning as needed, utilizing appropriate equipment.

Qualifications:

  • Proficient in Microsoft Office Suite (Outlook, Word, Teams, etc.).
  • Ability to learn and navigate case management software.
  • Familiarity with or willingness to learn about AAC (Augmentative and Alternative Communication) and assistive technologies.
  • Valid driver's license with a clean driving record.
  • Compliance with relevant background checks and pre-employment requirements.
  • Physical ability to perform tasks including lifting and transferring individuals, as this role may require significant physical activity.

Preferred Qualifications:

  • Experience applying person-centered techniques with individuals facing disabilities or other challenges.
  • Degree in a human services field with at least one year of experience working with adults with disabilities or brain injuries is preferred.
