Job Summary:

To coordinate and monitor care of patients in the assigned clinical area, ensuring seamless delivery of high-quality services. The ideal candidate will possess excellent leadership skills, with the ability to make informed decisions and prioritize tasks effectively.

Key Responsibilities:
  • Leadership and Coordination: Assist the manager in making monthly staffing schedules and day-to-day work assignments to department dialysis staff, ensuring optimal resource allocation and efficient workflow.
  • Workload Management: Assess workloads on an ongoing basis and make special assignments during peak and slow periods, ensuring that staff are utilized effectively and efficiently.
  • Staffing and Scheduling: Call in staff to meet minimum staffing requirements, making managers aware of requests for overtime, and coordinating with other clinical teams to cover staff shortages and process provider schedule changes when delegated by the manager.
  • Problem-Solving and Conflict Resolution: Handle difficult and problematic work situations, including resolving member issues and escalating to management as needed, ensuring that patient care is not compromised.
  • Training and Development: Serve as a resource for other staff members, answering questions, orienting new staff, and identifying staff needs for training, providing in-service training as needed to enhance skills and knowledge.
  • Special Projects: Perform special projects as assigned by the manager, including implementing new workflows and processes, removing barriers, communicating initiatives, researching, or identifying trends with patient-related data, etc.
  • Performance Management: Routinely provide feedback to the manager for staff performance, work habits, and barriers to efficient, high-quality work, ensuring that staff are held accountable for their actions.
  • Policies and Procedures: Assist with the development and revision of department policies and procedures, ensuring that they are aligned with organizational goals and objectives.
  • Operational Issues: Provide input into day-to-day operational issues that may support the hiring, evaluation, and termination of staff decisions, ensuring that decisions are made in the best interest of the organization.
  • Additional Responsibilities: Assess, plan, implement, and evaluate a plan of care for the peritoneal dialysis patient, including but not limited to the following functions:
    • Medication Administration: Administer all oral and injecting medications in the department, ensuring that patients receive the correct medications and dosages.
    • Narcotics and Drug Security: Ensure that narcotics and drug supplies are available in the assigned clinical area and assure the proper security of all narcotics and controlled substances according to KP standards and applicable regulatory laws.
    • IV Therapy: Initiate and monitor IV therapy to therapeutic and diagnostic procedures, ensuring that patients receive the necessary treatments and interventions.
    • Peritoneal Dialysis Exchanges: Initiate and monitor Peritoneal Dialysis Exchanges, including CAPD and APD, ensuring that patients receive the necessary treatments and interventions.
    • Emergency Preparedness: Provide ongoing assurance of functional emergency equipment and adequate emergency drug supplies, ensuring that patients receive the necessary care and interventions in emergency situations.
    • Documentation: Document emergency situations within the department to the clinical operations manager, ensuring that all necessary information is recorded and communicated.
    • Procedures and Safety Measures: Set-up, perform, and/or assist with procedures as appropriate, ensuring that patients receive the necessary treatments and interventions, and that safety measures are instituted for all compromised patients receiving care.
  • Patient Education: Coordinate, implement, and evaluate patient teaching plans for areas of diabetes mellitus, hypertension, anticoagulant therapy, therapeutic diet instruction, weight control, peritoneal dialysis at home, and other areas as designated, ensuring that patients receive the necessary education and support.
  • Walk-in Patients: Triage walk-in patients as needed or assigned, ensuring that patients receive the necessary care and interventions.
  • Patient Flow and Supervision: Supervise patient flow and direct other clinical support staff in the delivery of patient care within the Health Care Team (HCT), ensuring that patients receive the necessary care and interventions.
  • Patient Advocacy: Act as patient advocate and resolve problems within HCT, ensuring that patients receive the necessary care and interventions.
  • Unresolved Issues: Bring unresolved issues to the clinical operations manager's attention if unresolved within the team, ensuring that all necessary information is recorded and communicated.
Basic Qualifications:
  • Experience: Minimum six (6) months experience providing peritoneal dialysis self-care training, minimum one (1) year of current experience in a HMO or ambulatory setting, and minimum one (1) year experience in nursing care of the patient on home maintenance peritoneal dialysis or in nursing care of the end-stage renal patient.
  • Education: Bachelors degree in nursing.
  • License, Certification, Registration: This job requires credentials from multiple states. Credentials from the primary work state are required before hire. Additional Credentials from the secondary work state(s) are required post-hire. OR Compact License: Registered Nurse within 6 months of hire, Registered Nurse License (District of Columbia) within 6 months of hire, and Basic Life Support from American Heart Association.
Additional Requirements:
  • Competencies: Must complete all Fundamental and assigned Specialty related competencies by the end of the probationary period and annually thereafter.
  • Computer Skills: Proficiency in the use of applicable computer software required.
  • IV Therapy Experience: Experience in IV Therapy is required.
  • Reliable Transportation: Must have reliable transportation and a valid driver's license and able to complete a home assessment of patient's environment.
