We are seeking a skilled Licensed Practical Nurse to join our Home Care team at Hackensack Meridian Health. As a Home Care Nurse, you will provide high-quality patient care in the comfort of their own homes, focusing on one patient at a time.
Responsibilities- Provide nursing care to patients in their homes through assessment, development, implementation, and evaluation of care plans.
- Work under the direction of a registered nurse to provide home nursing care needs of assigned patients.
- Comply with physician orders in a timely manner and chart, audit, and demonstrate compliance on frequency and timely admission of referral.
- Assess patient acuity in a timely and accurate manner and communicate with healthcare team members through secure messaging.
- Prioritize and complete routine assignments consistently, accurately, and within guidelines.
- Complete patient care-related activities within scheduled frequencies and issue appropriate frequencies directed by physicians.
- Assure assessment, planning, intervention, and evaluation of patient care are reflective of policy, procedures, and standard of care.
- Teach patients and families based on identified health needs and chart, audit, and demonstrate documentation of teaching goals and plans.
- Report patients' problems to primary nurse or nursing management personnel with possible solutions.
- Communicate pertinent patient information and plan of care to primary nurse and obtain physician's orders as needed.
- Graduate of an LPN program or equivalent.
- Six (6) months of experience.
- Licensed Practical Nurse (LPN) certification.
- AHA Basic Health Care Life Support HCP Certification.
