Job Summary

The Orthopedic Care Navigator plays a vital role in facilitating the care of patients and the progress of the Orthopedic Total Joint Program. This position is responsible for coordinating the flow of patients and progress of the department, ensuring an optimum level of patient care and staff development.

Key Responsibilities
  • Conduct educational programs for patients and families, promoting a smooth joint replacement experience.
  • Coordinate patient processes from pre-op testing to post-op care, ensuring seamless transitions.
  • Develop and implement multidisciplinary care pathways, monitoring compliance and revising as needed.
  • Anticipate provider needs, preparing patients for surgery and ensuring a smooth surgical experience.
  • Assess patient needs, setting goals and implementing plans of treatment and education.
  • Market the orthopedic program through community outreach and education, utilizing patient feedback to improve care.
  • Monitor joint replacement clinical outcomes, identifying opportunities for improvement and growth.
  • Organize and lead the Orthopedic Program and Joint Center of Excellence, collaborating with multidisciplinary teams.
Requirements
  • Education: Bachelor's degree in Nursing
  • Certification: Registered Nurse (RN) - South Dakota Board of Nursing
  • Experience: 1+ years of healthcare experience, 1+ years of orthopedic experience, 1+ years of registered nurse experience
