Network Management Specialist

At Queens Community House, we are seeking a dedicated and compassionate Bilingual Case Worker to provide essential support to clients by helping them navigate challenges and achieve their personal goals.
Your primary focus will be improving clients' quality of life through individualized support plans, counseling, and referrals to community resources.
You will collaborate with a network of professionals to ensure clients receive holistic, high-quality care.
Key Responsibilities:
- Provide individualized support plans and counseling to clients
- Refer clients to community resources and services
- Collaborate with social service providers, healthcare professionals, and other community resources
- Develop and implement care plans to improve client outcomes
Requirements:
- Bachelor's degree in Social Work or a related field
- Fluency in both English and Spanish
- Master's degree in Social Work or related field
- Experience working with older adults
- Familiarity with community resources and support services
- Proficiency in case management software
