About the Role

We are seeking a skilled Clinical Liaison RN to join our team at Providence Home Services. As a key member of our care coordination team, you will play a vital role in ensuring seamless transitions between healthcare settings and providing exceptional patient care.

Key Responsibilities
  • Coordinate and arrange home care services between Providence Home Services and contracted facilities
  • Develop and maintain strong relationships with healthcare providers and patients to ensure effective care coordination
  • Collaborate with interdisciplinary teams to identify and address patient needs
  • Provide education and support to patients and families to promote self-management and independence
  • Participate in quality improvement initiatives to enhance patient outcomes and satisfaction
Requirements
  • Education: Bachelor's degree in Nursing or related field
  • Experience: 1 year of full-time experience in a clinical role
  • Licensure: Current RN license in the state of hire
  • Skills: Strong communication and interpersonal skills, ability to work in a fast-paced environment, and proficiency in electronic medical records
