Position Overview

In collaboration with the Site Clinical Director and the Senior Director of Population Health, the Clinical Care Team Leader is tasked with delivering both clinical and administrative guidance to the care management team at their designated site.

Key Responsibilities

  • Conduct comprehensive screenings and assessments for new and existing clients seeking various health services.
  • Work alongside the Senior Director of Population Health to evaluate data reports and coordinate outreach initiatives to meet client needs.
  • Engage with various internal and external providers to ensure holistic care is delivered effectively.
  • Oversee team operations to ensure that program goals and objectives are met within specified timelines.
  • Foster a collaborative environment by coordinating site operations with other leadership members.

Qualifications

Applicants should possess a Bachelor’s or Master’s degree in social work, along with a minimum of three years of relevant experience in a health-related, non-profit organization. Licensure as a Social Worker (LSW) or eligibility for licensure is required.

Leadership and Development

Provide mentorship and training to new and existing team members, enhancing their skills and fostering independence. Conduct individual and group supervision to ensure that essential competencies are developed and maintained.

Operational Excellence

Collaborate closely with the Site Clinical Director to ensure effective performance in direct service programs, including maintaining accurate documentation and record-keeping. Monitor service utilization and ensure timely access to care by coordinating with both internal and community resources.

Team Collaboration

Partner with the Site Clinical Director and other supervisors to track and report on program outcomes. Assist in managing daily clinical operations, including staff scheduling to meet productivity standards.

Additional Responsibilities

  • Facilitate team meetings and client care discussions.
  • Manage care transition notifications and assign outreach tasks as needed.
  • Engage with community providers to create a comprehensive service continuum for client success.
  • Participate in recruitment and hiring processes in collaboration with Human Resources.

This role is primarily office-based, requiring effective communication and collaboration with team members. Flexibility in working hours may be necessary for meetings and training sessions.
