Home Health Physical Therapist Position

Kaiser Permanente is seeking a skilled Physical Therapist to join our Home Health team. As a Physical Therapist, you will provide independent, age-appropriate physical therapy to patients in their place of residence, teaching patient/family and caregivers preventative and rehabilitative procedures, and developing and coordinating the plan of care and providing case management.
Responsibilities- Evaluate patients and interpret evaluation findings to determine the nature and extent of dysfunction
- Detect early warning signs of complications, including medical assessments of skin integrity and mentation
- Ability to differentiate emergent versus non-emergent patient care status and escalate appropriately
- Provide initial and ongoing assessment to determine level of functioning
- Evaluate the patient comprehensively, not just body part or functional ability
- Establish treatment goals and plans treatment to achieve established goals
- Initiate discharge planning for physical therapy
- Implement the plan of care through direct treatment of the patient and other healthcare personnel as appropriate
- Provide accurate, timely documentation of physical therapy patient management
- Participate in appropriate and approved educational and clinical research activities in physical therapy
- Complete OASIS documentation according to guidelines
- Coordinate with pharmacy, inpatient physical therapy, and outpatient departments
- Assist in the development, implementation, and review of departmental patient care programs
- Perform other duties as needed in the areas of staff education, peer reviews, departmental operations, and departmental projects
- Supervise the work of Home Health Aides and Physical Therapy Assistants according to regulation and agency policy
- Minimum of two (2) years of experience in physical therapy
- Graduate of an accredited physical therapy curriculum with a Bachelor's, Master's, or Doctoral degree with certification in Physical Therapy or foreign-trained equivalent
- Driver's License (California)
- Physical Therapist License (California)
- Basic Life Support
- National Provider Identifier
- Experience in an automated clinical information system
- One (1) year of experience in a home health agency
- OASIS Certification
