Position Summary:

We are looking for a Client Support Specialist (CSS) to join our multidisciplinary team, providing extensive case management services to individuals experiencing homelessness. The CSS will assist clients with various needs, including benefits (SSI, General Relief), housing, primary and other health care, and social and psychiatric services. In this role, you will guide clients through their treatment journey, identify barriers to care, and participate in treatment and discharge planning. Additionally, you will accompany clients to appointments as necessary and offer training and support in decision-making, vocational guidance, skills development, problem-solving, and crisis management.

Key Responsibilities:

  • Collaborate with a multidisciplinary care team to deliver comprehensive case management services, including outreach to individuals experiencing homelessness; engage in city-wide networks of case management; and foster relationships with care providers and AmeriCorps members.
  • Support clients throughout their treatment journey, whether in inpatient, outpatient, or community settings, ensuring they receive the education and resources they need; coordinate services, including benefits (e.g., SSI, Medicaid), housing, health care, and social services; and assist in developing crisis intervention plans.
  • Engage in quality improvement initiatives, focusing on care planning for high-risk patients and facilitating smooth transitions from medical and behavioral health facilities to ensure continuity of care.
  • Conduct outreach to clients in shelters, homes, day programs, and inpatient units as needed to facilitate access to services.
  • Participate in regional provider meetings and attend team and clinic staff meetings as required.

Qualifications:

  • 1-2 years of experience working with individuals experiencing homelessness, with a solid understanding of available services; strong problem-solving skills are essential.
  • A BA/BS degree is preferred, ideally in behavioral health or social services.
  • Excellent verbal and written communication skills in English are required; problem-solving skills are essential.
  • Bilingual proficiency in Spanish and English is strongly preferred, with additional compensation for demonstrated proficiency.

Compensation and Benefits:

  • Starting compensation is $22.07 per hour, with increases based on experience.
  • Full-time employees are eligible for a competitive time-off policy, including 4 weeks of vacation, health, dental, and vision insurance, a 403B retirement savings plan with employer contributions, and a pre-tax MBTA pass program offering a 40% discount. Eligible employees will also receive annual increases and an additional $7,500 for demonstrated bilingual proficiency.
