Job Summary

SSM Health is seeking a skilled RN Case Manager to join our team. As a RN Case Manager, you will play a critical role in ensuring patients receive the best possible care and support during their hospital stay and transition to the next level of care.

Key Responsibilities
  • Assess patients' discharge planning needs and develop individualized plans to ensure a safe and successful transition to the next level of care.
  • Collaborate with the interdisciplinary team to identify and address any barriers or gaps in care that may impact patient outcomes.
  • Utilize evidence-based practice and scientific knowledge to inform nursing practice and ensure that care is delivered based on patient-specific needs.
  • Participate in quality improvement initiatives to identify opportunities for improvement in standard work and contribute to the development of solutions.
  • May also be responsible for utilization management, ED case management, admission process, and liaison with post-acute providers.
Requirements
  • Graduate of an accredited school of nursing or equivalent education for licensure.
  • Three years' acute care experience.
  • Registered Professional Nurse (RN) licensure in the state of practice.
