Compassionate Hospice RN Case Manager

We are excited to offer a unique opportunity for a Compassionate Hospice RN Case Manager to join our team at Andwell Health Partners. As a dedicated RN, you will play a vital role in providing innovative and compassionate healthcare to patients in their own homes and communities.
The ideal candidate will have a current R.N. license in the State of Maine and one year of recent clinical nursing experience. ANA certification in Home Health or Community Health Nursing is encouraged.
In this full-time role, you will utilize your expertise to provide education and promote patient's rights and responsibilities for informed decision making in the plan of care, possess and utilize knowledge of pain control & symptom management, and demonstrate competency in assessment and technical skills.
