Job Overview

We are seeking a highly motivated and compassionate Behavioral Health Home Coor II to join our team at Northern Light Health. As a key member of our behavioral health delivery system, you will be responsible for providing comprehensive case management services to improve mental health and medical health outcomes for adults and children. This role requires strong communication and interpersonal skills, as well as the ability to work effectively in a fast-paced environment.

Key Responsibilities
  • Provide comprehensive case management services to improve mental health and medical health outcomes for adults and children.
  • Coordinate and facilitate access to preventative and health promotion services, mental health and substance abuse disorder treatment, and care coordination services.
  • Develop and maintain effective relationships with clients, family members, and other healthcare providers.
Requirements
  • Certified as a Mental Health Rehabilitation Technician (MHRT/C provisional or full certification), or certification eligible in a reasonable amount of time.
  • Valid vehicle operator's license in good standing with access to a vehicle insured with at least the minimum insurance covered by state law.
  • Bachelor's Degree in a closely related field or one year of full-time equivalent relevant human services experience.
Compensation

The estimated annual salary for this position is $62,000 - $72,000 depending on qualifications and experience.
