We are seeking a compassionate and experienced Peer Advocate to join our team at Wesley Health Centers' San Julian Oasis (SJO) Program. As a valued member of our interdisciplinary team, you will provide comprehensive health and social services to clients, collaborating with our health education and prevention team to share practical information and personal insights to assist clients facing similar challenges.
Program OverviewThe SJO Recuperative Care Program serves individuals experiencing homelessness and those who have previously experienced homelessness. Our program follows a Harm Reduction Model, and we are committed to supporting the success of each client. As a Peer Advocate, you will work closely with clients to build strong rapport, provide consistent support, and motivate them to engage in behavior modification programs.
Key Responsibilities- Establish and maintain strong relationships with clients, demonstrating genuine care for their well-being.
- Provide ongoing support and motivation to clients, encouraging them to engage in behavior modification programs.
- Accurately document client progress and maintain confidential records.
- Initiate regular contact with clients to monitor their progress and provide support.
- Accompany clients to referrals for health and social services, ensuring adherence and documenting progress.
- Facilitate referrals to substance use recovery services, healthcare facilities, family planning services, and other relevant social services.
- Deliver educational content and skill-building information to clients.
- Share personal experiences related to incarceration, substance use, mental health, and other risk behaviors.
- Contribute to the collection of program data and completion of program-related forms.
- Assist in the preparation of monthly reports for funding agencies.
- Proven experience working with individuals experiencing homelessness.
- High School diploma or GED.
- Exceptional communication skills, particularly in engaging homeless individuals with substance use or other related challenges.
- Personal experience overcoming challenges similar to those faced by clients.
- Ability to reliably travel to various community sites and attend appointments on time.
- A strong desire to make a difference in the lives of homeless individuals.
