POSITION SUMMARY:

The Child and Family Support Specialist fosters a strengths-based connection with children while delivering assistance, case management, and behavioral guidance. This role ensures that the family's perspectives and choices are honored and integrated throughout the treatment process. The specialist intervenes in instances of maladaptive behaviors, aiming to address the child's needs while preserving family integrity. This position requires both independent work and collaboration with other multidisciplinary team members.
PROGRAM DESCRIPTION:
The San Bernardino Wrap program is a community-oriented, family-focused initiative designed to support children with emotional challenges within their communities at the least intensive level of care. The program's objective is to minimize the risk of out-of-home placements and recidivism by uniting individuals, organizations, and the community as a collaborative decision-making team, with the primary focus on addressing the needs of the child and family. Services are aimed at meeting the mental health requirements of children and their families, facilitating safe and suitable care, and enhancing coordination and collaboration in care planning efforts with the Children and Family Team (CFT) and other child-serving organizations.
JOB DUTIES & RESPONSIBILITIES:
- Provide support to the child in home, educational, and community settings through a variety of case management services, including mentoring, coaching, and behavioral assistance.
- Identify and reinforce positive behaviors, activities, and skills demonstrated by the youth.
- Exhibit sensitivity to family culture, values, and beliefs, operating from a family-centered, strengths-based approach.
- Assist youth in acquiring and applying essential skills for harmonious interactions with both adults and peers across various environments.
- Facilitate transportation for youth to academic, social, recreational, and/or employment activities.
- Maintain accurate and thorough documentation in compliance with all program, contract, and payer standards.
- Collaborate with the client's multidisciplinary team, including county representatives, educational institutions, and external agencies, in the development and implementation of the treatment plan.
- Develop familiarity with community resources available to support children and their families.
- Participate in committees, lead training sessions, and represent the program at community events as necessary.
- Perform other related duties as assigned.
KNOWLEDGE, SKILLS & ABILITIES:
- Proficient in Microsoft Word, Excel, and Outlook.
- Demonstrated knowledge of crisis management techniques and the ability to apply them as needed.
- Ability to remain organized and manage multiple responsibilities simultaneously.
- Capable of establishing and maintaining appropriate rapport with children, youth, and families.
- Ability to work effectively as part of a multidisciplinary team.
- Understanding of behavioral modification strategies and the child welfare system.
GENERAL REQUIREMENTS:
Education/Experience:
- AA or Bachelor's degree in human services or a related field, and/or relevant experience working with youth in a mental health or social service environment, and/or two years of experience working with youth exhibiting challenging behaviors.
- Bilingual Spanish is strongly preferred.
Licensure, Registration, Certification:
- Valid driver's license and proof of automobile insurance.
- Completion of CPR/First Aid certification within the first thirty days of employment.
PHYSICAL REQUIREMENTS:
- Ability to drive a vehicle without restrictions, often in heavy traffic and frequently over long distances to ensure safe transport to client settings and appointments.
- Visual, auditory, and olfactory senses sufficient to operate a vehicle safely, identify environmental threats, and ensure client safety in the field.
- Occasional lifting and carrying of 10-20 pounds.
- Regularly required to sit, stand, climb stairs, and walk during family visits.
- Regularly required to sit and use a computer in office settings.
