Job Description

As an LPN Clinical Coordinator, you will work closely with our clients who identify as Trans + Non-Binary, providing them with expert care coordination and case management services. Your passion for serving this community is essential, and your experience in clinical care coordination or case management will be valuable assets in this role. While Spanish language ability is a plus, it is not required.

Required Skills and Qualifications

To succeed in this position, you should have:
  • An active LPN or RN license in the state of New York
  • Clinical care coordination or case management experience
  • A strong understanding of LGBTQ+ healthcare issues and concerns
