  • Collaborate with multidisciplinary treatment teams to identify and implement effective care strategies.
  • Utilize community resources and support services to provide referrals and promote patient well-being.
  • Develop and implement cost management strategies to reduce readmissions and improve patient outcomes.
  • Verify patient and family education and training, ensuring comprehensive understanding of care plans.
  • Stay up-to-date on regulatory standards and quality measures for participating payers.
  • Adhere to policies, procedures, and safety standards, completing required education assignments annually.
  • Participate in quality improvement initiatives and contribute to the achievement of departmental goals.
Requirements
  • Masters' degree from a CSWE-accredited graduate social work program.
  • Two years of experience in a clinical social work setting.
  • Licensed Clinical Social Worker (LCSW) certification.
