Job Description:

We are seeking a Healthcare Transition Coordinator to join our team at University Health. In this role, you will work closely with patients, families, and healthcare providers to ensure smooth transitions between care settings.

Key Responsibilities:
  • Conduct thorough assessments of patient needs and preferences
  • Develop and implement individualized transition plans
  • Coordinate services across multiple care settings
Requirements:

Graduation from an accredited school of professional nursing is required. Four years of clinical experience in service coordination, case management, or quality management is preferred. A current, unrestricted license to practice professional nursing issued by the State of Texas is also required.

Salary and Benefits:

Our estimated salary for this role is $65,000 - $80,000 per year, depending on experience. We offer a comprehensive benefits package, including medical, dental, and vision insurance, as well as paid time off and opportunities for professional growth and development.
