Responsibilities:

  • Care for patients with complex psychological, emotional, cultural, and social needs
  • Develop and implement individualized plans of care, incorporating evidence-based practices
  • Collaborate with interdisciplinary teams to ensure seamless care transitions
  • Analyze policy and standards documentation to ensure organizational compliance
  • Document patient history, symptoms, medication, and care provided
  • Assess learning needs and provide education to patients and caregivers

Requirements:

  • Bachelor's degree in Nursing (BSN) or related field
  • 1 year of experience in nursing, preferably in a hospital setting
  • RN License in state of employment or compact
  • Basic Life Support (BLS) certification
