Job Summary

We are seeking a skilled Nurse Service Coordinator to join our team at University Health. This is a challenging and rewarding role that requires strong clinical skills, excellent communication, and the ability to work effectively with patients, families, and healthcare professionals.

About the Role

The successful candidate will provide comprehensive care coordination services to our STAR Kids members, ensuring seamless transitions across the healthcare continuum. This includes assessing patient needs, developing individualized care plans, and coordinating access to medical, social, and community resources.

Responsibilities

  • Conduct thorough assessments of patients' physical, behavioral, functional, and psychosocial needs.
  • Develop and implement individualized care plans in collaboration with patients, families, and healthcare providers.
  • Cordinate access to acute and long-term services, including medical, social, and community resources.
  • Communicate effectively with patients, families, and healthcare professionals to ensure smooth transitions and optimal care outcomes.

Requirements

  • RN licensure with the Texas State Board of Nurse Examiners.
  • At least 5 years of clinical experience, preferably in service coordination, case management, or quality management.
  • Working knowledge of ICD-10, CPT4 coding, InterQual/M&R criteria, and Texas Department of Insurance HMO standards.

What We Offer

University Health offers a competitive salary range of $60,000 - $80,000 per year, depending on experience, plus excellent benefits, including health insurance, retirement savings plan, and paid time off.
