Hospice Clinical Liaison Specialist

The Hospice Clinical Liaison is a key member of our team, responsible for promoting the Compassus philosophy and values. This role requires strong communication and interpersonal skills, as well as the ability to work effectively with patients, families, and healthcare professionals.
Key Responsibilities- Attend and participate in staff meetings, educational programs, and community events to stay informed and network with colleagues.
- Communicate with referral sources to identify patients who may benefit from our services and provide education on our programs.
- Assess patient needs and work with the hospice team to develop personalized care plans.
- Collaborate with healthcare professionals to ensure seamless transitions and high-quality care.
- Provide education and support to patients and families to help them navigate the healthcare system.
- Stay up-to-date on industry developments and best practices to continuously improve our services.
- RN license and 2-3 years of related experience required.
- Bachelor's degree in Nursing highly preferred.
- Strong communication and interpersonal skills, with the ability to work effectively with diverse populations.
- Ability to work in a fast-paced environment and prioritize multiple tasks and responsibilities.
