Job Summary:

Mentor South Bay, a leading provider of behavioral health services, is seeking a dedicated and compassionate Family Centered Support Specialist to join our team. In this role, you will play a vital part in delivering high-quality support to children, adults, and families across Massachusetts and Connecticut.

Key Responsibilities:
  • Participate in Care Planning Teams to develop and implement personalized plans for identified youth and families.
  • Teach essential life skills, emotional regulation, and problem-solving strategies to enhance the well-being of youth and families.
  • Collaborate with Master's level Clinicians to implement treatment plans and monitor progress.
  • Identify and connect families with community resources to promote independence and self-sufficiency.
  • Maintain accurate documentation and adhere to company policies and procedures.

Requirements:
  • Bachelor's degree in a human service field and one year of experience working with children, adolescents, or transition-age youth.
  • Ability to work flexible hours, including afternoons and early evenings.
  • Valid driver's license and reliable vehicle.
