About the Role

We are seeking a skilled and compassionate Clinical Social Worker to join our team at Innovative Renal Care. As a key member of our dialysis team, you will play a vital role in providing exceptional patient care and support.

Responsibilities
  • Assess patient needs and develop personalized care plans to address physical, emotional, and financial demands of End Stage Renal Disease.
  • Collaborate with patients, families, and healthcare teams to ensure comprehensive care and support.
  • Provide education and guidance on patient rights, responsibilities, and disease management.
  • Conduct regular assessments and evaluations to monitor patient progress and adjust care plans as needed.
  • Develop and maintain strong relationships with patients, families, and healthcare teams to ensure seamless care coordination.
Requirements
  • Master's degree from an accredited school of social work (MSW).
  • Current state licensure (e.g. LCSW, LMSW) and meet all state required regulations to practice in a dialysis setting.
  • Two years of clinical work experience, preferably in an ESRD setting.
  • Current CPR certification preferred.
  • Excellent communication and interpersonal skills.
