About the Role

Kaiser Permanente is seeking a highly skilled Clinical Social Worker to join our team as a Disease Management Program Coordinator. In this role, you will play a critical part in managing disease management programs for at-risk patients with chronic psychiatric diseases or disorders.

Key Responsibilities
  • Coordinate an integrated systematic care management process to direct patients to timely specialty diagnostic and treatment care services.
  • Provide comprehensive on-going care management that meets established standards for efficiency, clinical outcomes, quality, resource utilization, and cost effectiveness.
  • Develop and implement clinical pathways and care management interventions using a multi-disciplinary team approach.
  • Analyze practice patterns and variations to establish care/treatment benchmarks.
  • Design a disease care model to address all components of patient care, including patient self-management, education, and stratified interventions.
Requirements
  • Minimum four (4) years of behavioral health case management experience in a relevant chronic disease in an acute medical center.
  • Masters degree in clinical social work from an institution accredited by the Council on Social Work Education.
  • Licensed Clinical Social Worker (California).
