Job Title: Substance Use Disorder Case Manager

At Harmony Recovery, we are seeking a highly skilled and compassionate Substance Use Disorder Case Manager to join our team. As a key member of our treatment team, you will play a vital role in helping individuals overcome substance use disorders and achieve long-term recovery.

Key Responsibilities:

  1. Develop and implement individualized treatment plans in collaboration with clients, addressing their unique needs and goals.
  2. Conduct regular meetings with clients to assess their progress, identify areas for improvement, and make adjustments to their treatment plans as needed.
  3. Collaborate with family members and other stakeholders to ensure a comprehensive understanding of the client's needs and to develop effective support systems.
  4. Facilitate communication between clients and healthcare providers to ensure seamless transitions and continuity of care.
  5. Participate in multidisciplinary treatment teams to share knowledge, expertise, and best practices in addressing substance use disorders.

Requirements:

  • Bachelor's degree in Psychology, Social Work, or a related field.
  • Experience working with individuals with substance use disorders and their families.
  • Strong communication and interpersonal skills, with the ability to work effectively with diverse populations.
  • Proficiency in Microsoft Office and electronic health records systems.
  • Certified Addiction Technician (CAT) designation or equivalent preferred.
