Job Title: Population Health Coordinator Assistant

Vancouver Clinic is seeking a dedicated and organized individual to join our Population Health team as a Coordinator Assistant.

This role provides administrative support to the Care Team by coordinating care for patients. Key responsibilities include scheduling appointments, gathering and reviewing patient records, scheduling preventive care appointments, and serving as a liaison between the patient, panel coordinator team, and staff.

Work Environment: Onsite at Evergreen Administrative Offices (13898 NE 28th St, Vancouver, WA 98682)

Work Schedule: Full-time, Monday-Friday, 8:00a to 5:00p

Compensation: Starting salary range $18.91, with potential for increase based on experience

Requirements:

  • High school diploma or equivalent required
  • Two years' experience in a healthcare environment providing direct administrative patient care required
  • Prior experience with Epic EHR system preferred
  • Knowledge of medical terminology
  • Effective communication and active listening skills
  • Demonstrated interpersonal and customer service skills
  • Philosophy and values consistent with a patient-centered care model
  • Ability to work proactively and effectively in a team and independently
  • Good organizational skills
