We are seeking a dedicated and compassionate Pediatric Healthcare Coordinator to join our team at Bradley Hospital. As a Pediatric Healthcare Coordinator, you will play a vital role in ensuring seamless transitions between hospital and community-based care, coordinating home visits, and providing education to patients and families.


Responsibilities
  • Coordinate home visits and provide education to patients and families on self-care strategies and disease management.
  • Collaborate with multidisciplinary teams to ensure comprehensive care and achieve optimal patient outcomes.
  • Maintain accurate records, document patients' progress, and communicate effectively with healthcare providers.

Requirements

To be successful in this role, you must have:

  • A current Registered Nurse license in the State of Rhode Island or licensure in accordance with the Nurse Licensure Compact agreement.
  • At least one year of experience in pediatric nursing.
  • Excellent communication and interpersonal skills, with the ability to work effectively with patients, families, and healthcare professionals.
