Job Summary:A Home Health Nurse, or Home Health RN, is responsible for traveling to a patient's home to administer their services and helping patients maintain their independence. The ideal candidate will have a strong clinical background, excellent communication skills, and a passion for delivering high-quality patient care.
Key Responsibilities:- Patient Care:
- Perform initial and ongoing patient assessments to determine home care needs.
- Develop and revise individualized care plans based on patient assessments.
- Administer medications and treatments as prescribed.
- Provide health care instructions to patients and caregivers.
- Facilitate discharge planning to ensure continuity of care.
- Regularly re-evaluate patient nursing needs.
- Communication:
- Prepare clinical notes and update the primary physician regularly.
- Communicate effectively with physicians and community health resources to coordinate care plans.
- Supervise and evaluate home health aide care every fourteen days.
- Ensure that arrangements for equipment and other necessary items and services are available.
- Promote health and independence across the lifespan through our skilled services and education of patients, families, and caregivers.
- Qualifications, Skills, and Competencies:
- Graduate of an accredited school of nursing.
- 1-2 years of recent patient-centered care/nursing experience.
- Current state licensure, CPR certification, and a valid driver's license.
- Bachelor's degree in nursing with one year of home health care experience preferred.
- Strong observation, communication, and problem-solving skills.
- Physical capability to lift patients and equipment, with considerable walking, standing, reaching, bending, and kneeling.
