Job Summary:

A Home Health Nurse, or Home Health RN, is responsible for traveling to a patient's home to administer their services and helping patients maintain their independence. The ideal candidate will have a strong clinical background, excellent communication skills, and a passion for delivering high-quality patient care.

Key Responsibilities:
  • Patient Care:
    • Perform initial and ongoing patient assessments to determine home care needs.
    • Develop and revise individualized care plans based on patient assessments.
    • Administer medications and treatments as prescribed.
    • Provide health care instructions to patients and caregivers.
    • Facilitate discharge planning to ensure continuity of care.
    • Regularly re-evaluate patient nursing needs.
  • Communication:
    • Prepare clinical notes and update the primary physician regularly.
    • Communicate effectively with physicians and community health resources to coordinate care plans.
    • Supervise and evaluate home health aide care every fourteen days.
    • Ensure that arrangements for equipment and other necessary items and services are available.
    • Promote health and independence across the lifespan through our skilled services and education of patients, families, and caregivers.
  • Qualifications, Skills, and Competencies:
    • Graduate of an accredited school of nursing.
    • 1-2 years of recent patient-centered care/nursing experience.
    • Current state licensure, CPR certification, and a valid driver's license.
    • Bachelor's degree in nursing with one year of home health care experience preferred.
    • Strong observation, communication, and problem-solving skills.
    • Physical capability to lift patients and equipment, with considerable walking, standing, reaching, bending, and kneeling.
