At CenterWell Home Health, we're committed to delivering exceptional care to our patients. As a Home Health Occupational Therapist, you'll play a vital role in helping patients recover from illness, injury, or surgery, and regain their independence.

  • Assess patients' daily living and work-related skills, and develop personalized therapeutic retraining programs to achieve measurable objectives.
  • Collaborate with physicians and other healthcare team members to develop and revise treatment plans that meet each patient's unique needs.
  • Provide hands-on therapeutic treatment and instruction to patients, helping them improve strength, coordination, range-of-motion, and function, or teach compensation measures.
  • Review and expand retraining programs to ensure they align with patients' behavioral goals and the overall rehabilitation program.
  • Consult with other vocational team members to develop and implement vocationally oriented plans that cater to patients' needs and capabilities.
  • Communicate patient status and progress to physicians, rehabilitation staff, Clinical Managers, patients, and family members.
  • Participate in care coordination and discharge planning activities, and serve as a resource to other healthcare personnel in meeting patients' needs.
  • Design community reintegration activities to support patients' physical reconditioning and psychological adjustment, and coordinate these efforts with the interdisciplinary team.
  • Recommend and design special adaptive equipment to enhance patients' living or working environments, or improve their participation in the rehabilitation program and community.
  • Train staff and family members to carry out retraining programs, including dressing, feeding, grooming, and hygiene skills, and participate in team and family educational meetings.
  • Supervise, instruct, and evaluate Occupational Therapy Assistant and Home Health Aide performance in implementing occupational therapy services.
  • Accurately document patients' care observations, interventions, and evaluations, ensuring that interim verbal orders from physicians are accurately documented and implemented.

Requirements:

  • Degree from an accredited Occupational Therapy Program
  • A minimum of six months of occupational therapy experience preferred
  • Home Health experience a plus
  • Current and unrestricted OT licensure
  • Current CPR certification
  • Good organizational and communication skills
  • A valid driver's license, auto insurance, and reliable transportation are required.

Compensation and Benefits:

The compensation range for this position reflects a good faith estimate of starting base pay for full-time employment. The pay range may be higher or lower based on geographic location and individual pay will vary based on demonstrated job-related skills, knowledge, experience, education, certifications, etc.$76,800 - $105,800 per year
