Salary
$21.913 per hour

Job Description

In this rewarding role, you will serve as a case manager for individuals who have been determined eligible for the Aged and Disabled Waiver or the Developmental Disabilities Day Waiver or Comprehensive Waiver. Your responsibilities will include:

• Coordinating and overseeing the delivery of services
• Conducting assessments and eligibility determinations
• Developing plans of services and supports
• Providing appropriate referrals
• Monitoring provision of services
• Ensuring health, welfare, and safety of those we support

Required Skills and Qualifications

To succeed in this position, you will need a Bachelor's degree in education, psychology, social work, sociology, human services, or a related field, along with professional experience in services or programs for individuals with intellectual or other developmental disabilities. Consideration will be given to applicants who will graduate within 90 days of application.
