About the Role:

VNS Health is seeking a highly skilled and compassionate Registered Nurse to lead our Hospice Care team. As a Nurse Manager, you will be responsible for overseeing the delivery of high-quality end-of-life care to patients and their families in the comfort of their own homes.

Key Responsibilities:

  • Lead a dedicated interdisciplinary team in providing comprehensive hospice care services, ensuring the highest level of patient satisfaction and quality of care.
  • Develop and implement clinical and quality improvement initiatives to enhance patient outcomes and meet regulatory requirements.
  • Collaborate with the Quality Improvement and Education teams to ensure professional practice standards are consistently met.
  • Manage the day-to-day operations of the department, including effective caseload management and achievement of productivity and revenue targets.
  • Participate in Hospice Team meetings, Interdisciplinary group management meetings, family meetings, and patient calls to ensure seamless care coordination.

Requirements:

  • License and current registration to practice as a Registered Professional Nurse in New York State.
  • Bachelor's degree in nursing from an approved program accredited by the National League of Nursing or Health or Human Services field.
  • Minimum three years of clinical nursing experience, preferably in a home care or hospice setting.
  • Clinical management experience is preferred.
