Overview

At Christian Horizons, we're seeking a skilled MDS Coordinator to join our team. As a key member of our care services team, you'll play a vital role in ensuring the highest quality of care for our residents.

Responsibilities
  • Assess resident care needs and develop individualized care plans
  • Schedule and conduct resident care conferences to coordinate care across departments
  • Conduct MDS assessments and CMI's as required, ensuring timely and accurate submissions
  • Review medical records and collect information for MDS records
  • Evaluate resident care and identify areas for improvement
  • Direct and organize required resident assessments and documentation for reimbursement
  • Ensure quality customer service and foster a positive environment for residents and staff
Requirements
  • Graduate from an accredited school of nursing
  • Current state license as an LPN or RN
  • Clear understanding of MDS and experience with MDS preferred
  • Able to follow and give written and oral directions
  • Demonstrate dependability, cooperation, and interest in the care of the elderly
