Key Responsibilities
  • Provide nursing care to patients in their homes, following proper nursing techniques and Agency policies.
  • Communicate effectively with healthcare teams, patients, and families to ensure seamless care coordination.
  • Document patient visits and reports, and order necessary supplies.
  • Assess patient needs and develop care plans based on age-specific requirements.
Requirements
  • MA LPN license and completion of all annual licensure requirements.
  • Excellent interpersonal, judgment, and communication skills.
  • Documentation skills and ability to identify patient situations requiring MD and/or RN follow-up.
