CompensationThe estimated salary for this position is $90.00 or $95.00 per completed Assessment, depending on the county of operation. This competitive compensation reflects the importance of your role in delivering high-quality care to our patients.
Job DescriptionWe are seeking a skilled Nurse Practitioner to join our team of healthcare professionals. As a Home Risk Assessor, you will conduct comprehensive health assessments in the homes of our patients, gathering crucial information about their medical history, current health status, and social environment. This vital work enables us to identify potential health risks and develop targeted interventions to improve patient outcomes.
As a key member of our team, you will work closely with patients, families, and other healthcare professionals to ensure seamless care coordination. Your expertise and compassion will make a tangible difference in the lives of our patients, empowering them to take control of their health and well-being.
Requirements and QualificationsTo be successful in this role, you will need:
- A Master's Degree in Nursing or a related field, or equivalent experience and satisfactory completion of NP licensure.
- Current RN and NP licensure in the state of practice, including prescription authority or the ability to obtain prescriptive authority.
- Board certification by a recognized nursing organization, such as the AANP, ANCC, or AACCN, in a Matrix-approved specialty.
- A current BLS, ACLS, or CPR certification.
- A minimum of 1 year of experience as a Nurse Practitioner.
- Strong computer skills and familiarity with Employee Health/Medical Record software.
- Excellent verbal and written communication skills, with the ability to work effectively with patients, families, and colleagues.
- The ability to travel to patient locations, with a valid state driver's license and adequate automobile insurance coverage.
