Nurse Practitioner Home Health Specialist

As a Nurse Practitioner Home Health Specialist, you will be an integral part of our team, working collaboratively with healthcare professionals to provide comprehensive care to patients in the comfort of their own homes.
Key Responsibilities- Conduct thorough assessments of patients' health, medical history, and social environment to identify potential risks and develop personalized care plans.
- Collaborate with primary care physicians, case managers, and other healthcare professionals to ensure seamless care coordination and optimal patient outcomes.
- Provide education and support to patients and their families to promote self-management and healthy lifestyle choices.
- Document patient interactions, assessments, and care plans accurately and efficiently to ensure continuity of care.
As a Nurse Practitioner Home Health Specialist with Matrix Medical Network, you will enjoy a competitive compensation package, including a base pay of $90.00 or $95.00 per completed assessment, depending on the county. You will also have the opportunity to work flexible schedules, including days, evenings, nights, and weekends, to accommodate your personal and professional needs. Our PRN/Part-Time providers are non-benefited, except for 401(k) and limited voluntary benefits, and we offer a mileage reimbursement plan to support your travel needs.
Requirements and QualificationsTo succeed as a Nurse Practitioner Home Health Specialist with Matrix Medical Network, you must possess a Master's degree in Nursing or a related field and be a current RN and NP licensee in the state of practice. You should also be board-certified by a national certifying organization, such as the AANP, ANCC, or AACCN, and have a minimum of one year of experience as a Nurse Practitioner. Strong computer skills, excellent verbal and written communication, and the ability to work independently and collaboratively are essential for this role.
Travel and LocationAs a Nurse Practitioner Home Health Specialist with Matrix Medical Network, you will travel to patients' homes, skilled facilities, and other community environments in Ashtabula, Ohio. You will have the flexibility to work in various settings, including home visits, skilled facility visits, and telehealth visits, and will be required to drive a car to meet patients where they live.
