Job Summary

GuideStar Eldercare seeks a skilled Licensed Professional Clinical Counselor to provide neurobehavioral health services to residents in long-term care facilities. As a key member of our clinical team, you will work collaboratively with specialty clinicians to deliver holistic, continuity of care to elderly individuals with neurocognitive disorders and other neurologic and/or psychiatric conditions.

Key Responsibilities
  • Provide neurobehavioral health services to residents in long-term care facilities
  • Collaborate with specialty clinicians to deliver holistic, continuity of care
  • Partner with geriatric care specialists to develop and implement individualized care plans
  • Conduct comprehensive assessments and develop treatment plans to address residents' neurobehavioral health needs
  • Communicate effectively with residents, families, and healthcare teams to ensure seamless care coordination
Requirements
  • Master's degree in Counseling
  • Current licensure as an LPCC in the state of employment
  • Excellent communication and interpersonal skills
  • Ability to work collaboratively as part of a multidisciplinary team
  • Strong analytical and problem-solving skills
