Home Health Nurse Practitioner Opportunity with Matrix Medical Network

Job SummaryWe are currently seeking a highly skilled and compassionate Home Health Nurse Practitioner to join our team of healthcare professionals in Ashtabula, OH. As a Home Health Nurse Practitioner with Matrix Medical Network, you will have the opportunity to make a meaningful difference in the lives of our patients and their families by providing high-quality, patient-centered care in the comfort of their own homes.
Responsibilities- Conduct comprehensive health assessments, including reviewing medical history, medication adherence, social environment, and other risk factors, to identify potential health issues and develop effective care plans.
- Develop and implement treatment plans, including medication management, health education, and follow-up care, to improve patient outcomes and quality of life.
- Collaborate with healthcare teams, including physicians, social workers, and family members, to ensure seamless care coordination and communication.
- Provide patient education and support to promote healthy behaviors, disease management, and self-care.
- Document patient encounters and outcomes accurately and efficiently, ensuring compliance with Matrix Medical Network's policies and procedures.
- Master's degree in Nursing with a focus on Nurse Practitioner studies.
- Current RN and NP licensure in the state of practice, with prescriptive authority.
- Board certification by the AANP, ANCC, or AACN in a Matrix-approved specialty.
- Minimum of 1 year of experience as a Nurse Practitioner, with a strong foundation in home health care and patient-centered care.
- Excellent communication and interpersonal skills, with the ability to work effectively with patients, families, and healthcare teams.
- Strong organizational and time management skills, with the ability to prioritize tasks and manage multiple patients and responsibilities.
The salary for this position is $90.00-$95.00 per completed assessment, with the opportunity to earn additional income based on the number of completed assessments per month.
