This is a full-time position.
Flexibility Required: Employees must be adaptable to work any of our 8-hour shift schedules during standard business hours with a 1-hour lunch break. Occasional overtime may be necessary based on business needs.
Role Overview: This position involves fieldwork, primarily visiting various facilities.
Key Responsibilities:
- Assist the APC(s) in the overall coordination of member care, following their guidance.
- Collaborate effectively as part of a team.
Data Management Tasks Include:
- Managing member charts for new members, post-hospital visits, and health maintenance updates.
- Entering lab results and medication information.
- Obtaining necessary medical records and consult notes.
- Supporting appointment scheduling and transportation arrangements.
- Facilitating member education and building relationships with facilities.
Specific Tasks for Skilled Nursing Facility Members:
- Track skilled days and assist with completion of necessary documentation.
Specific Tasks for Assisted Living Facility Members:
- Assist members in utilizing their benefits and transitioning to skilled nursing facilities.
- Coordinate home health services and durable medical equipment.

Required Qualifications:
- High School Diploma / GED or equivalent work experience.
- Must be at least 18 years old.
- Minimum of 18 months of clinical or medical record experience.
- Proficiency with electronic medical records (EMRs) and Microsoft Office Suite.
- Reliable transportation for travel to client sites.
- Ability to work full-time and travel as needed.
Preferred Qualifications:
- Experience with CMS compliance.
- Certification as a Nursing Assistant or Medical Assistant.
Essential Skills:
- Strong communication abilities.
- Excellent organizational and administrative skills.
- Capability to manage multiple tasks effectively.
- Experience in cross-functional team environments.
Compensation: The hourly rate for this position is competitive and based on various factors, including local labor markets and individual qualifications.
