Primary Care Opportunity

We are seeking a Full Time Nurse Practitioner to join our team at our Outpatient clinic in Waterbury, CT. The schedule is Monday-Friday 8a-5p; no nights and no weekends.

What We Offer:

  • Competitive Salary: Ranges from $110,000 - $165,000 based on experience and location, with quarterly bonus based on quality metrics

Required Qualifications:

  • National Certification: In at least one of the following specialties: Family Nurse Practitioner, Adult-Gerontology Primary Care Nurse Practitioner, Adult Nurse Practitioner, Gerontological Nurse Practitioner
  • State License: Active, non-probationary state Nurse Practitioner License
  • Experience: 1+ year direct primary care experience as a Nurse Practitioner
  • DEA License: Active

Role Summary:

  • Provision of Exceptional Primary Care
  • Conduct Office Visits: Routine and acute issues, and home visits as needed
  • Annual Wellness Visits: Administer health risk assessments
  • Care Coordination: With other providers, specialists, testing facilities, and agencies
  • Population Health Leadership: In coordination with the Care Team
  • Phone Triage and Outreach: Assisting the care team
  • Patient Education: On their health conditions, care plans, and treatments
