Bienvivir All-Inclusive Senior Health is seeking a highly skilled and compassionate Social Worker to join our team. As a Social Worker, you will play a vital role in providing outreach, social services, crisis counseling, and intervention to our participants and their families.
Responsibilities:- Provide case management services to an assigned caseload of participants, ensuring their medical, physical, emotional, and psychosocial needs are met.
- Conduct home safety screens, complete plans of care, and facilitate smooth transitions for participants.
- Collaborate with the multidisciplinary team to diagnose problems, formulate treatment plans, and evaluate progress.
- Provide advanced directives education, assist with completion, and participate in ongoing communication with participants and their families.
- Establish and maintain positive relationships with contracted assisted living, foster homes, and nursing home facilities.
- Advocate for participants and their families with agencies such as Social Security Administration, Health and Human Services Commission, and Adult Protective Services.
- A graduate of an accredited university with a master's degree in social work and licensed by the Texas State Board of Social Worker Examiners.
- One year's experience in providing social services to a frail or elderly population is preferred.
- Knowledge and experience working with the geriatric population and family systems, as well as community referral systems and case management.
- Must be bilingual (Spanish/English).
