Job Summary:

As a Senior Social Services Specialist with the DEPARTMENT OF STATE HEALTH SERVICES, you will play a critical role in providing highly advanced social services and case management work to families in need. Under the direct supervision of a Program Supervisor and overall direction of the Program Manager, you will perform senior-level social services and case management work, including case management for children and pregnant women, newborn screening, childhood lead poisoning prevention program, personal care services, children's health insurance program, child protective services, and case management services for families in the children with special health care services program.

Key Responsibilities:

  • Provide case management services to families in need, including conducting comprehensive assessments, developing service plans, and providing follow-up services.
  • Assist families with the eligibility determination process for the children with special health care services program.
  • Travel to conduct home visits and provide case management services to families in their homes.
  • Collaborate with community groups, other regions, public health entities, and regional program staff to promote efforts to service families with children who have health risks, conditions, and special health care needs.
  • Participate in community events related to case management needs and provide presentations and information to stakeholders and other community members as needed.

Requirements:

  • Bachelor's degree in social work or a related field.
  • At least 2 years of experience in social services or case management.
  • Excellent communication and interpersonal skills.
  • Ability to work independently and as part of a team.

Working Conditions:

This position requires frequent travel to conduct home visits and participate in community events. The ideal candidate will have a valid driver's license and a reliable vehicle.
