About the Role:

  • The Patient Care Navigator will be responsible for coordinating patient care across the healthcare continuum, working closely with providers, peers, and community colleagues to ensure seamless transitions and optimal outcomes.
  • This individual will conduct psychosocial assessments and develop interventions to address patients' unique needs, promoting patient-centered care and community engagement.

Key Responsibilities:

  1. Cross-continuum care coordination
  2. Psychosocial assessments and interventions
  3. Collaboration with healthcare teams and community partners
  4. Identification of opportunities to improve patient care and services
  5. Leadership and initiative in promoting best practices

Requirements:

  • Master's Degree in Social Work (MSW)
  • Licensed in the state of Washington (LICSW or LSWAIC)
  • 3+ years of experience in hospital or equivalent settings
  • Health industry knowledge and leadership experience preferred
