Job Title: Community Skills Specialist

Join Constructive Partnerships Unlimited as a Community Skills Specialist and play a vital role in empowering individuals to achieve independence, inclusion, and productivity.

Job Summary:

As a Community Skills Specialist, you will be responsible for implementing and monitoring individual program plans for participants served. You will work closely with participants, their families, and other stakeholders to ensure that each individual receives the support and resources they need to thrive.

Key Responsibilities:
  • Monitor activities of assigned participant group and facilitate activities that promote independence, individualization, inclusion, and productivity.
  • Provide functional training to maximize participants' independence through person-centered planning.
  • Assist in the development and implementation of participant program plans.
  • Participate in case conferences and all participant-related meetings.
  • Assess participants' skills, needs, and preferences and implement community-based training opportunities.
  • Maintain accurate and contemporaneous documentation, including dates sheets and inclusionary logs.
  • Prepare monthly notes and provide supervisors with data sheets and required paperwork.
  • Review submitted documentation for accuracy and completeness.
Requirements:
  • Associate Degree or 60 college credits in a healthcare-related field.
  • Minimum 1 year of experience in a related field.
  • Successful completion of AMAP, CPR, and other required training, including PrecisionCare and basic MS Word.
  • NYS Driver's License preferred.
  • Ability to work outdoors for community-based activities.
Essential Physical Requirements:

The ability to coordinate the movement of the arms, legs, and torso together in activities where the whole body is in motion, including the ability to exert oneself physically over long periods of time.

The ability to safely lift individuals to/from various surfaces and perform a two-person transfer.

The ability to push/pull, turn, and maneuver equipment, including occupied wheelchairs.

Possess strength, coordination, and agility to safely and quickly respond to emergency situations.

Working Conditions:

Works in a fast-paced environment with multiple priorities and deadlines.

May be required to work outdoors for community-based activities.

May be required to lift, bend, stretch, rotate, or reach out with the body, arms, and/or legs to ensure the safety of individuals in all situations.
