About the Role:

We are seeking a highly motivated and compassionate Certified Family Peer Support Partner to join our team at the Institute for Community Living. As a Certified Family Peer Support Partner, you will play a vital role in providing support and guidance to families enrolled in our CCBHC program.

Key Responsibilities:

  • Provide outreach and support to family members at every stage of the program, from pre-admission to discharge.
  • Work collaboratively with parents/legal guardians to advocate, parent, and care plan in a strengths-based manner.
  • Provide psychoeducation, skill building, and participate in home visit planning and processing.
  • Create opportunities for families to support each other through group meetings and activities.
  • Provide warm hand-off referrals to community-based support services.
  • Actively participate as an integral part of the CCBHC multidisciplinary team, including family care planning and collaboration.
  • Perform basic accountability tasks related to the job, including compiling and maintaining necessary records.
  • Assist administrative staff in collecting information for program effectiveness.
  • Actively engage in clinical supervision.
  • Respond in a positive and respectful manner to diversity.
  • Participate in all trainings for the Whole Family Health Model.

Essential Knowledge, Skills, and Abilities:

  • Knowledgeable in peer-based practice.
  • Able to connect with people living with mental illness, and/or substance abuse.
  • Knowledge about mental health, physical wellness, and stress management, as well as local resources that promote health and wellness.
  • Welcoming, guiding, and empowering representative of the program.
  • Model a strength-based, family-driven approach.
  • Attend trainings to continually increase competencies.
  • Demonstrated commitment to the value of diversity, equity, and inclusion.
