About the Role:

We are seeking a skilled Case Management Professional to join our team at Special Service for Groups, Inc- HOPICS. As a key member of our staff, you will be responsible for providing supportive services to individuals experiencing homelessness. Your primary goal will be to help clients achieve stability and independence through housing placement and coordination.

Responsibilities:

  • Conduct eligibility screenings, service coordination, and triage clients for additional housing services needs.
  • Develop and implement individualized housing plans, working closely with clients to identify their unique challenges and goals.
  • Collaborate with other project staff and partners to provide housing placement services and supportive services.
  • Maintain accurate records and documentation in accordance with funding requirements and agency policies.
  • Attend meetings and training sessions as required, staying up-to-date on industry developments and best practices.

Requirements:

  • An associate degree in social services or a related field is required; relevant work experience may be substituted.
  • A minimum of one year of job-related experience working with homeless individuals and families is preferred.
  • Strong communication and interpersonal skills, with the ability to work effectively with diverse populations.
  • Proficiency in Microsoft Office and database management software.

Salary: $25.50 - $26.50 per hour
