At Care Synergy, we are seeking a compassionate and skilled Licensed Practical Nurse (LPN) to join our team as a Hospice Palliative Care LPN. As a member of our interdisciplinary team, you will provide high-quality patient care and support to individuals and their families in the comfort of their own homes.

Key Responsibilities:
  • Provide patient care and support in accordance with applicable scope and standards of practice, keeping within the policies, values, and mission of the organization.
  • Collaborate with RN Case Manager, Charge RN, or assigned RN in the coordination of care to ensure Plan of Care (POC) is being followed.
  • Attend IDG/IDT Meetings as assigned.
  • Provide continuous monitoring of patient's physical, emotional, and spiritual care needs.
  • Communicate patient and family information, status, and level of care to RN, manager, provider, and other IDT members.
  • Assist with coordination and successful completion of patient transfers.
  • Educate patients, families, caregivers, and community regarding hospice philosophy, palliative, and home health treatments.

Requirements:
  • Graduate of an accredited school of practical nursing.
  • One year of experience in hospice, palliative care, and/or home health, medical/surgical, public health, oncology, acute care, or home health.
  • Current Colorado LPN license.
  • CPR certification.
  • Valid Colorado driver's license and proof of automobile insurance.
