Head of Care Coordination and Oversight

The Head of Care Coordination and Oversight is responsible for providing administrative leadership for the Tracking team. This role will collaborate with Regional Directors, Practice Managers, Providers, clinical personnel, and external facilities to ensure efficient tracking and coordination of patient care. The utilization of computerized databases and electronic records will facilitate the necessary tracking and care coordination.
Location and Work Schedule
This is a salaried position within the Tracking and Care Coordination Department.
