Job Summary

We are seeking a highly motivated and compassionate Case Manager to join our team at TFI Family Services, Inc. As a Case Manager, you will play a critical role in supporting children and families in achieving their goals and improving their overall well-being.

Key Responsibilities
  • Work collaboratively with children and families to identify and address their needs and goals.
  • Develop and implement individualized plans to support families in achieving their goals.
  • Provide education and support to families on topics such as emotion regulation, healthy relationships, and conflict resolution.
  • Connect families with community resources and services to support their needs.
  • Collaborate with other professionals, such as social workers and therapists, to ensure comprehensive support for families.
Requirements
  • Bachelor's degree in Social Work or related field.
  • Experience working with children and families, preferably in a case management or social work setting.
  • Strong communication and interpersonal skills.
  • Ability to work independently and as part of a team.
  • Flexibility and adaptability in a fast-paced environment.
