The School-Based Care Coordinator is a key member of the Lamoille Health Partners Clinical Enterprise and Behavioral Health & Wellness teams. This role integrates with local schools to provide prevention, early identification, and intervention strategies for behavioral health conditions.

**Key Responsibilities**

  • Collaborate with school-based therapeutic resources to coordinate students' plans of care and support goal achievement.
  • Provide healthy lifestyle counseling, education for chronic conditions, and general case coordination for high-risk, socially complex students.
  • Identify and manage unmet needs of students to ensure appropriate resources are in place to improve outcomes.
  • Support families by connecting them to community resources requested by schools to ensure participation and remove barriers.
  • Integrate care partners in various settings to address barriers for social determinants of health, medical care, and related behavioral health factors that affect health and well-being.

**Requirements**

This role requires strong communication and collaboration skills, as well as the ability to work effectively with diverse stakeholders, including students, families, and school staff.
